I have been involved on something similar. My approach was to document it as a case study making it as generic and anonymous as possible with made-up companies, actors etc.

And I fudged some of the minor details.

It was much like a no-blame postmortem.

Also avoid pointing fingers, being judgemental etc.

Come at it from a completely neutral PoV.
